American Youth Football and Cheer requires that each player is certified.  Below is a list of the required documents. Please be sure to bring all the printed registration forms that were provided electronically during the online registration process to one of our uniform fitting/paperwork nights. If you did not print the forms when you registered, simply log back into: www.login.siplay.com and print all 9 forms available. Thank you for your effort and cooperation with the collection of these documents.


The following items are required (forms are printed via our online registration system):

  • Registration Fee (paid online)

  • AYF Participation Tracking and ID Card Page 1 Signed and initialed

  • AYF Participation Tracking and ID Card Page 2 Signed and initialed

  • Emergency Medical Treatment Consent

  • Medical Clearance signed by participants Doctor or Annual Physical that MUST be dated after January 1st of the current year

  • Image Release

  • Waiver and Release of Liability

  • Code of Conduct

  • Concussion Acknowledgement

  • Absentee Form -Signed but NOT dated

Additionally, AYFC requires:

  • Original Birth Certificate with a raised seal
  • Copy of Final current year’s Report Card, name, date and school year must be visible
  • Health Form evidencing a current year's physical (n/a if Medical Clearance form - above - has been completed & signed by your doctor).

NOTE: Health Forms evidencing an examination during the previous year will NOT be accepted by the league, even if they are not yet a year old.  You must have your doctor sign off on the Medical Clearance form instead.
